I need you to please complete this Nursing Patient Plan of care base on my patient medical Diagnosis of Alzheimer’s disease. I need you to follow all the instructions and if have concerns please ask me.I will attached all the information from the patient, including medications.

In the Goals please create a long within a month and short goal within a week for that patient who is at risk for injury that must be realistic and measurable.
