Project description
Our health is greatly influenced by factors other than what happens inside a doctor’s office. Where we live, work and go to school are also factors that affect our

health. Communities need leadership and action to create solutions that result in better health for the people who live there.

The Community Health Project you have worked on throughout the 10 weeks of your first course, was designed to help you look closer at the people who live in a specific

community and think about what they need to live healthier lives. The Windshield Survey allowed you to observe the community through a specific filter and better

understand the environment. The Gap Analysis required you to research and identify the health concerns that are likely to be found in the population living in the

community, and then determine if the health care services that exist in the community are sufficient to aIDress those concerns. Finally, based on your observations and

research, you were asked to recommend new or aIDitional health care services that will lead to better health for the community.

Your completed Community Health Project should include the following:

Title page.
Abstract (one paragraph).
Windshield Survey template and summary (12 pages).
Gap analysis (12 pages).
Recommendations (12 pages).
Reference page (minimum of 3 resources).
Appendixannotated bibliography (12 paragraphs for each of your 4 resources).

Your Community Health Project should be approximately 5 pages in length (not including the title page, reference page, and appendix) and include references to support

your analysis and recommendations from at least 4 scholarly or professional resources. Be sure you follow APA guidelines throughout.
