BEHAVIOR CHANGE THEORIES PAPER ASSIGNMENT

Your assignment is to become familiar with one behavior change theory and write about it. Select one (1) of the behavior change theories presented in the text. In making your choice, however, be sure that you will be able to fulfill the assignment.
The theory paper should contain the following components (in this order):
1. Name and describe the theory
Using full APA citations, name and fully describe the theory. If the theory lends itself to visual modeling, present that model along with a brief description of each component of the model. Limit the length of this section of the paper to no more than one page of text. The model may be presented on an aIDitional page.
2. Describe how the chosen theory has been used in health education
Using full APA citations, cite two examples from primary sources in the literature where the chosen theory was used to support a health education initiative. Briefly describe each project/program and give evidence of the application of the theory. Limit the length of this section of the paper to no more than one page.
