YA works as a Family Nurse Practitioner and TA is a licensed practical nurse. YA works with a
private agency while TA is employed at a nursing home in Illinois. YA hours was reduced due to
covid -19 and he sometimes works from home doing paperwork. Due to impact of covid-19 on
their family, T A who used to work PRN now got a full-time job at nursing home where she works
night shift. TA is a nurse, mother, wife and a small business owner. YA helps with house chores
and takes care of their son when TA is at work.
Demographic Data and identifying data:
AFRICAN AMERICAN
YA- 43 years old, TA-40 years married for 6 years. YA has master’s in nursing and TA has
diploma in nursing and full-time employment as nurses
JA- 4 years old, kindergarten
Development stage of family members and history of family:
Middle-aged parents and school-aged children
YA was born in Illinois to immigrant parent from west Africa, his brothers and sister are all living
in the united states but different states. YA has a great health history no health issues and no
past medical history or surgeries. YA both parents are alive and healthy with no apparent health
issues.
TA was born in west Africa, moved to the USA 10 years ago, 2 of her sisters are living in the UK
before moving to the USA she was living in the UK with her sister. Her Dad is late but mother still
living, her dad died a natural death. Her mum has hypertension and every other health history
are normal. TA suffered from infertility, they had to removed fibroid from her womb before she
was able to conceive her son, apart from that she has no health problems.
JA- is a healthy 4 years old, with no health issues or developmental delay.
Environmental data:
Living in a single-family household, good neighborhood surroundings, their son is enrolled in a
Daycare, but stays at home now due to the ongoing pandemic. They have some family members
living in Illinois and the others out of state. And their home is in a great shape.
Family functions/family roles:
YA- Works from home and his hours was reduced due to Covid-19
TA- working as a full-time nurse at a nursing home 8 days in 2 weeks and 8 hours overnight
shift. When she’s not at work, she takes care of her son, do house chores and cook, YA also
help with house chores. And she also run a small hair business.
JA- is at home with the mum during the day, the mum helps teaching him some alphabets and
numbers following a curriculum, and the dad take care of him at night.
Family stress and coping: on their off days they go out to have family time, staying active. Their
communicate a lot which makes things more easier, even though their financial aspect is really
bothering them due to YA reduced work hours, but they hope everything will soon balance out

1. Describe the strengths of community as evidenced by Windshield Survey.
This community is close knit with lowest crime rate, peaceful and hardly will you hear any noise,
very quiet. Residents living in this area are nice, peaceful and respectful. This area is dominated
by mostly Caucasians, then some Hispanics and African Americans. The school district might not
be the best but it’s great.
2. Describe the weaknesses (gaps in service) as evidenced by Windshield Survey.
The school district needs some changes, and some roads need the cycling lane, mostly you see
bikes ridding side by side with car which is dangerous. And More traffic lights need in the area.
3. Identify 1 problem based on the identified gap in community resources as an indicator of potential
poor health outcomes
Inadequate traffic light at the back roads that leads to the corn fields, a lot a accident occur on
this side of the neighborhood especially during winter period.
